At Elko County, the health and safety of our employees is among our top priorities.  As part of our commitment to a healthier, happier workforce and with the rising cost of healthcare, providing a wellness program is important. 

As a result, employees and spouses covered on the health plan who choose to participate in the wellness program (health screening and questionnaire) will receive a 20% plan premium discount as well as a $200 deductible credit (for both employee and spouse)!  Some employees took advantage of the opportunity to participate in the onsite screenings during the health fair last week.  However, if you did not attend the health fair, we are providing you an additional opportunity to participate in the program through October 31, 2014 – which will enable you to receive the 20% plan premium discount as well as a $200 deductible credit! 

 Please be reminded if you have Employee and Spouse coverage or Employee and Family coverage, your spouse is required to participate in the health screening as well as complete the health questionnaire in order to receive the premium discount and deductible credit.     

 In order to participate, you (and your spouse, if necessary) need to take the attached form – SimplyWell Physician Upload Form – to your personal physician.  Your personal physician will need to complete the required information on the form and send it to SimplyWell via fax or mail no later than October 31, 2014 in order to be eligible for the reduced premiums and deductible credit.  All of the required areas on the form must be complete in order to receive credit for participating.  Please do not return the form to Elko County. 

In addition, you and your spouse must also log on to www.simplywell.com to complete the online health questionnaire in order to receive credit for participating.  If you need assistance logging in to your account, please contact Human Resources or SimplyWell 1.877.991.9355 at as soon as possible.
